Patient Portal
Nowak Aesthetics uses a Patient Portal in order to better facilitate information between you and our practice. If scheduling through our website, you will receive an email from one of our friendly staff members shortly after your appointment has been confirmed. If scheduling in our office or over the phone, you will receive a text shortly after your appointment has been confirmed. In this email or text, you will find your log-in security code along with helpful information to walk you through creating your portal account.
We ask that you use the Patient Portal to remotely complete all the required paperwork needed for your appointment PRIOR to your visit.

Office Policies
Financial Policy
Thank you for choosing Nowak Aesthetics for your cosmetic needs. Our primary mission is to provide our patients with outstanding medical care. Your clear understanding of our Patient Financial Policy is important to our professional relationship.
Appointments require deposits at the time of booking services. Appointments must be secured with a minimum 25% deposit, unless specifically communicated otherwise. Certain appointments require payment to be made in full prior to treatment. Appointment deposits serve to reserve our medical staff and equipment specifically for you and your treatment. Appointments that are scheduled with a treatment time of less than one hour, a minimum 48-hour notice is required to cancel and/or reschedule. Appointments that are scheduled with a treatment time greater than one hour, a minimum 72-hour notice is required to cancel and/or reschedule. If this notice is not provided you will be subject to a fee of 50% of the total treatment’s value. Any outstanding late fees on your account will need to be paid at the time of your next re-booking. We will do everything we can to accommodate late arrivals, however, if we do not have enough time to provide quality treatment without impacting other patient appointments, we may need to cancel your appointment and reschedule. Late arrival cancellations will forfeit the appointment deposit.
All service purchases expire 12 months after the date of purchase and are non-refundable and non- transferrable. All products must be picked up within 14 days after payment or they expire. Any promotional items (Free products included in any bundle) are final sale. All monthly promotions are final sale and Free procedures must be rendered within 3 months of promotion purchase date.
All sales are final. This includes purchases of products and services as well as appointment deposits. If you no longer wish to use your appointment deposit towards the intended treatment, it will become a credit, good for 12 months from the date of purchase, for use towards other products and services.

Appointments
Scheduling
Please contact our office to schedule your appointment and to provide us with your up-to-date information. You may also request an appointment here on-site and one of our patient coordinators will be in contact with you.
Should I arrive early?
We ask that you arrive 15 minutes early to allow enough time for the check-in process. One of our friendly staff members will help check you in and guide you through the visit.
Payments
We accept Cash, All Major Credit Cards, Apple Pay, Care Credit Financing, Patient Fi Financing, Alle Cherry Financing, and valid Nowak Aesthetics Gift Cards. Payments must be made in full at the time services are rendered.
Health Insurance
If you are insured by a PPO insurance plan or have a health savings account, please request a copy of the check-out form at the end of your visit. This form can be used to submit a claim to your insurance carrier for direct reimbursement. Please check with your plan administrator or insurance carrier website for information regarding your policy’s benefits.
